In reference to the article regarding the 2009 Membership Directory that was in our most recent eNewsletter, we would also like to offer the opportunity for our PPO members who have multiple branch offices to list those accordingly in the directory. Please fill out the attached form and fax it to our office at (916) 930-0552 or send via email to mrazo@calsaga.org. This will ensure that the most updated contact information for each branch is reflected in our directory. It will also allow us to update your information in our database. The time table to respond does not change. Please send this to our office no later than close of business – April 10, 2009.
